SUNSCREEN WAIVER

give permission for the Bloomfield teachers / counselors to apply / reapply sunscreen to

if and when needed whilst on the school premises or when on a school field trip.

Sunscreen will only be applied by a teacher when the child is in the presence of another teacher / counselor.
